What it checks
For nursing homes, the federal requirements for aide in-service training (42 CFR 483.95(g) and 483.35(d)(7)):
- At least 12 hours of in-service training a year for every nurse aide.
- Dementia management and resident abuse prevention training included.
- Care of the cognitively impaired for aides who care for those residents.
- Training that addresses weaknesses found in each aide’s performance review.
- A performance review of every aide at least once every 12 months.
For home health agencies (42 CFR 484.80(d)): at least 12 hours of in-service training in each 12-month period, supervised by a registered nurse, with records kept to show it.

Questions
- How many hours of in-service training does a nurse aide need?
- At least 12 hours a year. In nursing homes the rule is 42 CFR 483.95(g), and the training must include dementia management and resident abuse prevention. Home health aides need at least 12 hours in each 12-month period under 42 CFR 484.80(d).
- Does the ledger provide the training itself?
- No. It records whatever training you already give, from any course provider or your own staff educator, and shows who still needs hours or required topics.
- What does the survey report show?
- For each aide: the training year, hours completed, the required topics covered and missing, and the next performance review date, followed by every session with its date, hours, topics, trainer, supervising RN and attendees. Print it or download it as a CSV file.
- How is the price counted?
- $39 a month covers one site with any number of aides, up to 500. A second building or agency is a second subscription. Cancel any time from your account page; access continues to the end of the month paid for.
- Does it cover state rules for assisted living?
- Not yet. The ledger applies the federal rules for nursing homes and home health agencies. You can log any other training and topic, but state-specific hour rules are not checked.
